Become a New Patient

We are currently accepting new patients into our practice. Thank you for considering us. To become a new patient:

  1. Make an Appointment
  2. Sign up for our patient portal
  3. Download your patient forms online through the patient portal

When you come to our office for the first time as a new patient, we’ll ask you to complete some initial forms, including an Authorization and Consent for Treatment form, if you were not able to download them from the patient portal in advance of your appointment.


What to Expect at Your First Appointment

You’ve made your appointment to get established as a new patient. Now what?

Be sure to arrive at your appointment at least 15 minutes ahead of your scheduled time. This allows the office to collect all of your paperwork, create your chart and enter all of your information into our electronic system.

New patients are encouraged to print out and complete the below forms before arriving for their appointment. This will speed up your check-in process. If you are unable to print out our forms, we ask that you please arrive 30 minutes before your appointment time to fill out your forms in our office.

Other patient information to bring includes:

      • Photo ID
      • Insurance Card
      • List of current medications with dosage and frequency(prescription and over-the-counter as well as supplements)
      • Copies of medical records from previous doctors
      • History of surgical procedures with dates
      • Any current or recent test results or x-rays

If you have medical records that need to be transferred from a previous care provider, please complete our Authorization to Release Medical Records form as well. We will submit that request on your behalf and have your records sent directly to our office.

When you enter the office, please sign in at the receptionist desk. Provide the staff member your completed paperwork. There may be other forms to sign at that time.

When you are called, you will be accompanied into our patient care area. An assistant or nurse will review your paperwork and chart with you for accuracy. Initial vital signs will be checked and the reason for your visit will be discussed. The assistant will exit the room and speak with the doctor. You will then be seen as quickly as possible by your new medical provider.

Common Courtesy is Your Expectation and Our Policy

When you make an appointment, you expect to be seen by your care provider within a reasonable amount of time. If a delay occurs, you appreciate being updated as you wait or given an opportunity to reschedule if necessary. These are just a few common courtesies we provide our patients.

Patient Forms

Preferred Contacts (PDF) - Patients are encouraged to complete and return the Preferred Contacts Form but it is not required. Contactos Preferidos
